用管內撐開法取出和 Pacemaker Wire 沾黏的 Perm-Cath

這是一個愛爾蘭 Sligo University Hospital 的 case report, 這個 case 的 Perm-Cath 可能因為久放，外面的 fibrin sheath 和 pacemaker 的 wire 還有 SVC 沾黏住，對於這類沾黏住的 Perm-Cath，Joon Ho Hong 在 2011 年提出了一個方法，用 balloon 在 catheter 的 lumen 裡 dilatation, 把 lumen 撐大、拉扯 catheter wall、把 fibrin sheath 撐碎。他們運用了這個方法在沒有影響到 pacemaker 的功能下成功的 exchange Perm-Cath。

這個案例我們可以學到：

1. Endoluminal balloon dilatation 把 fibrin sheath 撐碎，這種技巧叫 Hong’s technique。
2. 用的是 0.018 V18 wire 和 6 mm x 4 cm low-profile balloon (一般 balloon 進不去), 打到 14 atm。
3. 放了兩年的 Perm-Cath 有 22% 可能會卡住。
4. 其他可能的解決方案：有種 laser sheath 可以在 distal end 放出 laser 把 fibrin sheath 破壞，而且不會傷到 vessel（感覺有點貴），不過它最大內徑是 12.5 Fr（Perm-Cath 的外徑可能是 14–15 Fr），使用上有所限制。
